New Princeton IV Consensus Updates Guidelines on Erectile Dysfunction and Cardiovascular Health

PASADENA, Calif. — August 8, 2024 — A new international consensus paper co-authored by Dr. Robert A. Kloner, Chief Science Officer and Scientific Director of Cardiovascular Research at Huntington Medical Research Institutes (HMRI), provides updated clinical guidance on the management of erectile dysfunction (ED) in men with cardiovascular disease (CVD).

Published in Mayo Clinic Proceedings, the article — “The Princeton IV Consensus Recommendations for the Management of Erectile Dysfunction and Cardiovascular Disease” — represents the latest installment in the long-standing Princeton Consensus series, a multidisciplinary collaboration between cardiologists, urologists, endocrinologists, and sexual medicine experts.

The Fourth Princeton Consensus Conference was held March 10–11, 2023, at Huntington Medical Research Institutes in Pasadena, California, continuing the tradition of uniting global experts to align evidence-based approaches for men with ED and cardiovascular risk.

Key updates include:

  • Reinforcing that men presenting with ED should be evaluated for underlying cardiovascular risk until proven otherwise.

  • Introducing coronary artery calcium (CAC) scoring as an advanced tool for cardiovascular risk stratification.

  • Refining treatment algorithms for phosphodiesterase type 5 inhibitors (PDE5i) — the standard oral therapy for ED — and highlighting emerging evidence of their potential cardioprotective benefits.

“These recommendations represent an important bridge between sexual medicine and cardiovascular prevention,” said Dr. Kloner. “ED can serve as an early indicator of cardiovascular disease, and timely evaluation may save lives.”

The paper also underscores the value of multidisciplinary collaboration — the hallmark of the Princeton Consensus — in ensuring safe, effective, and personalized care for men with both cardiovascular and sexual health concerns.
